A client reports poor sleep and variable energy. A prominent screen marker appears worrying, but several reports come from the same scan and the client’s functional experience does not clearly agree with the display.
Your task: Map client report, clinical context, scan observations and response over time. Separate literal observations from interpretation, flag dependent evidence and uncertainty, then defend one priority, a proportionate action and a reassessment measure.
